Traffic Crash Information

 

You have been involved in a traffic crash:

All traffic crashes involving property damage or injuries must be reported to the police immediately. The Hawthorn Woods police officer investigating this this traffic crash is providing you with the information you need. Check to make sure the information is correct.


The Motorist Report Form contains the following information:

Date, time, location and Hawthorn Woods Police Department traffic crash report number, as well as the name, address, telephone number and insurance policy number of the other driver(s) involved.

What should you do now?

Contact your insurance agent as soon as possible - there may be a time period restriction for you to make your claim.

Have the above information handy for him/her, as this will help speed up processing of the claim). If damage to any vehicle in the traffic crash exceeds $500.00 (estimated) or there is an injury, each driver is required by state law to complete the Motorist Report form and mail within 10 (ten) days to:

Information about your traffic crash report:

There may be blank boxes on your report form. These are for internal use or do not apply to your traffic crash.
Police officers are not required to complete narratives or diagrams on all traffic crash reports. Narratives and diagrams are only prepared on injury traffic crashes or traffic crashers requiring the tow of one or more vehicles.

What about tickets issued by the police officer?

If the other driver receives a traffic ticket:
you do not have to appear in court unless notified by the Hawthorn Woods Police Department.

If you receive a traffic ticket:
Two copies of a ticket are issued.

    Violators copy - Yellow
    Explains the violation. Used as receipt for bond (i.e. driver's license, bond card, cash posted for bond). Instructions on how to handle on the back.

    Court Communication Copy - Blue
    Must be used to make payment.
    Must be used to request trial date.

There are basically three options with a traffic ticket:

Traffic tickets are not issued for traffic crashes occurring on private property, including parking lots (except for D.U.I. or Reckless Driving).

Where do you get copies of the traffic crash report?

Copies of completed reports are usually available five business days after the traffic crash. You can pick up a copy at the Hawthorn Woods Police Department Records Section. Office hours are Monday through Friday 8 am to 4 pm. There is a $5.00 charge for each copy.
